# NEUROBEHAVIORAL STATUS EXAM {#nse}
## Notification/Informed Consent
{{< var first_name >}} {{< var last_name >}} consented to undergo the current neuropsychological evaluation as part of a work-up for mental health diversion. {{< var mr_mrs >}} {{< var last_name >}} was informed that none of what we discussed would remain confidential, and information {{< var he_she >}} provided during our meeting and the test results from my evaluation of {{< var him_her >}} would become a part of my report, to be used at {{< var his_her >}} attorney's discretion. {{< var he_she_cap >}} was informed a copy of any report I produced would be sent to {{< var his_her >}} attorney, possibly to the prosecuting attorney, and to the court. {{< var mr_mrs >}} {{< var last_name >}} indicated having understood the purpose of the evaluation and the limitations of confidentiality of our meeting and the testing I conducted. The evaluation was conducted at XX Detention Center, CITY, CA lasting approximately TIME hours.
This report is based on a review of available information contained in medical records and criminal records, consultation with attorney {{< var attorney >}}, and information gathered during the evaluation.
I reserve the right to change my opinions about the following should I be provided additional records that alter my understanding of the findings.

## Reason for Referral
{{< var first_name >}} {{< var last_name >}}, a {{< var age >}}-year-old {{< var handedness >}}-handed male with a {{< var education >}}-year education, was referred for neuropsychological evaluation. Currently, he is incarcerated since DATE, following a CRIME, which led to charges of X, Y, and Z. The purpose of this evaluation is to determine {{< var mr_mrs >}} {{< var last_name >}}'s eligibility for mental health diversion versus long-term imprisonment, given his history of severe and prolonged episodes of psychotic mania leading to illegal behaviors.
## Background
### Developmental/Medical History
Biggie Smalls was born in 1999 in CITY, STATE, to biological parents. He grew up in a middle-class family with two siblings, achieving normal developmental milestones without any childhood mental health diagnoses. In school, he excelled academically, earning recognition as "most likely to succeed" and "most intelligent" in high school. Notably, he preferred solitary activities, such as reading, over socializing. {{< var mr_mrs >}} attended USC, dropping out after three years before eventually graduating with a degree in applied mathematics. His family medical history includes intellectual disability and schizophrenia in relatives.
