Add staging and grading terminology for Japanese Classification system

Detailed Description

The Japan ARGO program (GMAC-JP) uses a different staging system to denote the clinical and pathological TNM categories, stage groups and tumour grades. The program lead (Tatsuhiro Shibata) sent a reference to this classification system so I will work on adding the new terminology to the dictionary.

Possible Implementation

• Add Japanese Classification System to clinical_tumour_staging_system, pathological_tumour_staging_system, recurrence_tumour_staging_system and posttherapy_tumour_staging_system
• Update controlled terminology to ensure T, N and M category values are included in the *_t_category, *_n_category and *_m_category fields
• Update controlled terminology in *_stage_group fields to include new stage groups
• Update controlled terminology in tumour_grade field to include new tumour grades

There are a number of additional clinical parameters unique to this classification system, so I'm in the process of confirming which additional fields they intend to submit (will create separate ticket if we need to add new fields).
