"Add Another" publication url button should not be enabled on pristine Project Info section

Steps to reproduce:

1. Create a new application
2. Navigate to the Project Info section.
3. Do not make any modifications to the form. The "Add Another" button below the publication urls fields is enabled, even though there are no existing urls added to the default 3 inputs.

This button should be enabled only if 3 valid publication urls have already been added. It should also be disabled if there is an extra field added, but it has not yet been filled, or contains errors.

Also remove the unnecessary mandatory red asterisk in the "Add Another" text input boxes
