Ottehr - Intake - Current Medications/Allergies/Medical History/Surgical History Screens - Add Free Text Option

Description:
In the Current Medications, Allergies, Medical History, and Surgical History screens, users currently have to select from predefined options only. It should be possible to enter free text in these sections to allow for details not covered by the existing options.

Steps to Reproduce:

1. Navigate to each of the following screens within the intake flow:
   • Current Medications
   • Allergies
   • Medical History
   • Surgical History
2. Attempt to add an entry not listed in the predefined options.

Expected Result:
Users should be able to enter free text in addition to selecting from predefined options, enabling them to provide custom information relevant to each section.

Actual Result:
There is currently no option for free text entry, limiting users to predefined options only.
