Description:
When patients attach PDF files for school or work notes on the "Do you need a school or work note?" screen, the files initially appear as uploaded. However, if the user proceeds to the "Complete consent forms" screen and then navigates back to the "Do you need a school or work note?" screen, an error message is displayed, and the PDFs are not uploaded as expected.

Steps to Reproduce:

1. Start Ottehr intake flow
2. Navigate to the "Do you need a school or work note?" screen.
3. Attach PDF files for school and/or work notes.
4. Click "Continue" to proceed to the "Complete consent forms" screen.
5. Click the back button to return to the "Do you need a school or work note?" screen.

Expected Result:
The attached PDF files should be successfully retained and displayed without any errors when returning to the screen.

Actual Result:
"Failed to upload card. Please try again" error message is shown for the previously attached PDFs, and the console logs an error: "Mixpanel token is not set."
