Items stuck in workflow_actions

Descriptive summary

As of today, items getting stuck in approval process.

Expected behavior

Click approved --> item moves to Deposited, as advertised on show page and all other pages.

Actual behavior

I leave a note and click approve and get a white screen with the item stuck in a workflow_actions URL:

And then it appears the item is approved — listed as Deposited — but it still shows as Under Review when filtering on the Works page:

HOWEVER, it is not listed on the Review Queue page -- that's all correct.

Steps to reproduce the behavior

1. Approve item
2. Get white page

@simholt I found a way to update these to become active in their state on being deposited and I tested on staging & local and this method works, so I'm going in to update these four works and after that you can review it and close this ticket. I think it is a problem with a sometime thing with workflow, since it is only affecting these 4 works, I'm just going to update them manually and if the problem persist down the future, I think we would have to look into it more when it arise.
