Feedback form not implemented correctly

Feedback is going into LIBWEBF JIRA queue - is this the best place? should Hours have its own JIRA queue?

• Feedback topic is "Feedback from SearchWorks" - change to "Library Hours feedback"
• Feedback form should open above the red banner, as it does in SearchWorks
• clicking Send does send the feedback, but does not close the form or indicate that the submit has been successful - so users are clicking multiple times. Again, see SearchWorks for correct behaviour: Send should close the form, remain on the same page, and add a flash alert: "Thank you! Your feedback has been sent."
