misc server browser improvements

- Fixed an issue where an incomplete response from one master can abort the refresh prematurely on engines that query multiple masters automatically.
- Fixed various bugs in favorite server handling.
- Increased max servers from 128 to 512.
- Misc tweaks and code cleanup.
