v1.3 Wrap-up

A final PR for v1.3 (#372) meant to finish up a final few tasks:

• Replace all hard-coded FPGA addresses with the defines given by xparameters.h  #386
• Update REV D & E Block Designs for v1.3 #361
• Remove driver docs from firmware repo (they are now in docs.amdc.dev)
• Delete motherboard from IP repo

I have confirmed that the Vivado Hardware definition successfully builds in the SDK with the BSPs and app projects for all three hardware targets.
