A simple PHP form that allows users to submit work requests that are emailed to the recipient.

Prerequisites

• PHP 4
• Email address

To use

1. Add the work.php file to a directory of your choosing.
2. Open the file and modify the top few lines to your selected options.
3. Share link to whoever

• Add &mobile to the end of the URL to display the form at a larger size and fit to the screen.
